Auto Insurance Principles Should Apply to Health Insurance

Many Americans rely on their automobiles to get to work. No automobile means no job, no rent or mortgage money, no food. A single parent, struggling to make ends meet in the suburbs with 100,000 miles on the odometer, would presumably welcome the guaranteed opportunity for low-priced insurance that would take care of every possible repair on her auto until the day that it reaches 200,000 miles or falls apart, whichever comes first. Especially if the insurance is valid regardless of whether she even changes the oil in the interim.

So why aren’t the auto insurance companies writing such coverage, either directly or through used auto dealers? And given the importance of reliable transportation, why isn’t the public demanding such coverage? The answer is that both auto insurers and the public know that such insurance can’t be written for a premium the insured can afford, while still allowing the insurers to stay solvent and make a profit. As a society, we intuitively understand that the costs associated with taking care of every mechanical need of an old automobile, particularly in the absence of regular maintenance, aren’t insurable. Yet we don’t seem to have these same intuitions with respect to health insurance.

If we pull the emotions out of health insurance, which is admittedly hard to do even for this author, and look at health insurance from the economic perspective, there are several insights from auto insurance that can illuminate the design, risk selection, and rating of health insurance.

Auto insurance comes in two forms: the traditional insurance you buy from your agent or direct from an insurance company, and warranties that are purchased from auto manufacturers and dealers. Both are risk transfer and sharing devices and I’ll generically refer to both as insurance. Because auto third-party liability insurance has no equivalent in health insurance, for traditional auto insurance, I’ll examine only collision and comprehensive insurance — insurance covering the vehicle — and not third-party liability insurance.

Bumper to Bumper

The following are some commonly accepted principles from auto insurance:

* Bad maintenance voids certain insurance. If an automobile owner never changes the oil, the auto’s power train warranty is void. In fact, not only does the oil need to be changed, the change needs to be performed by a certified mechanic and documented. Collision insurance doesn’t cover cars purposefully driven over a cliff.

* The best insurance is offered for new models. Bumper-to-bumper warranties are offered only on new cars. As they roll off the assembly line, automobiles have a low and relatively consistent risk profile, satisfying the actuarial test for insurance pricing. Furthermore, auto manufacturers usually wrap at least some coverage into the price of the new auto in order to encourage an ongoing relationship with the owner.

* Limited insurance is offered for old model autos. Increasingly limited insurance is offered for old model autos. The bumper-to-bumper warranty expires, the power train warranty eventually expires, and the amount of collision and comprehensive insurance steadily decreases based on the market value of the auto.

* Certain older autos qualify for additional insurance. Certain older autos can qualify for additional coverage, either in terms of warranties for used autos or increased collision and comprehensive insurance for vintage autos. But such insurance is offered only after a careful inspection of the automobile itself.

* No insurance is offered for normal wear and tear. Wiper blades need replacement, brake pads wear out, and bumpers get dings. These aren’t insurable events. To the extent that a new car dealer will sometimes cover some of these costs, we intuitively understand that we’re “paying for it” in the cost of the automobile and that it’s “not really” insurance.

* Accidents are the only insurable event for the oldest automobiles. Accidents are generally insurable events even for the oldest autos; with few exceptions service work isn’t.

* Insurance doesn’t restore all vehicles to pre-accident condition. Auto insurance is limited. If the damage to the auto at any age exceeds the value of the auto, the insurer then pays only the value of the auto. With the exception of vintage autos, the value assigned to the auto goes down over time. So whereas accidents are insurable at any vehicle age, the amount of the accident insurance is increasingly limited.

* Insurance is priced to the risk. Insurance is priced based on the risk profile of both the automobile and the driver. The auto insurer carefully examines both when setting rates.

* We pay for our own insurance. And with few exceptions, automobile insurance isn’t tax deductible. As a result, the fear of increasing insurance rates due to traffic violations and/or accidents changes our driving behavior and we sometimes select our automobiles based on their insurability.

Each of the above principles is supported by solid actuarial theory. Although most Americans can’t describe the underlying actuarial theories, most everyone understands the above principles of auto insurance at the intuitive level. For sure, as indispensable automobiles are to our lifestyles, there is no loud national movement, accompanied by moral outrage, to change these principles.

Unsustainable Market

In contrast, similar principles are routinely violated in health insurance. To demonstrate this, let’s return to the same suburban mother from the opening paragraph. She’s busy working, driving to and from work, and driving her kids to school and activities. She ends each day exhausted, sitting on the couch with fast food. She’s obese, has a sedentary life, a bad diet, and hasn’t taken the time to go to the doctor in years. After a simple injury doesn’t heal for weeks, she turns up at the emergency room and learns she has type II diabetes. Although type II diabetes is controllable, changing diet and exercise habits and properly tracking her condition takes time and effort and she’s never quite successful in implementing the necessary lifestyle changes.

So the initial emergency room visit is only the first of a long list of health care related to non-controlled diabetes and other problems associated with obesity. Whether she has individual or group insurance, her insurance pays for each episode of care, without singling her out for a premium increase, and without charging her any more cost sharing than is charged to the healthiest and most medically diligent insureds. Her coverage continues until she voluntarily changes insurance companies and/or employers or becomes eligible for Medicare. If she’s covered under group insurance she may not even pay any premium. Her insurance continues unabated, even though the disease was caused by neglecting her body and she maintains her poor lifestyle even after the disease becomes known.

This just wouldn’t happen in auto insurance. This scenario is the auto insurance equivalent of guaranteed access to low-priced auto insurance that takes care of every possible repair, including damage already done, until the day the car falls apart so completely it’s unsalvageable (death) or reaches 200,000 miles (Medicare), regardless of whether she even changes the oil (takes care of herself) in the interim.

As a society, we don’t expect this in private-market auto insurance, but we expect it in private-market health insurance. Furthermore, there’s a chorus of national and state interests, which continuously pushes us further away from the auto insurance principles.

The current private health insurance market isn’t sustainable. Prices have been consistently increasing faster than inflation for decades. Each year, insureds use more health care than ever before and more people have no insurance at all. Most actuaries and other people in the private health insurance market don’t want national health insurance with its bureaucracy and one-size-fits-all benefits. Yet, we’re trying to sustain a private insurance system, which violates the very principles we know are necessary for private insurance markets.

Yes, health insurance involves the sacredness of human life and is therefore different from auto insurance. But if we’re to sustain a private-market solution to health insurance, actuaries need to explain to the larger society, in terms that society understands, the rationale for the following principles:

* As sacred as health care is, it’s still an economic transaction that has to be balanced by individuals and societies, against other economic choices. It can’t be unlimited. Sometimes it will be secondary to other choices. On a given day, for example, the mother in our scenario may value her car more than her health.

* Insurance premiums should be paid by the individual and tied to controllable risk factors. This will provide the best incentive for the control of risk factors.

* Although it’s hard to draw the line between abuse, neglect and ignorance, self-abuse shouldn’t be insured and we need to draw that line somewhere.

* The private market can’t provide unlimited, self-directed health insurance.

* Routine care and ongoing treatments of chronic conditions can be pre-funded, can even be subsidized, but they don’t constitute “insurable events.”

* Insurance can’t be expected to keep every human body in pristine condition. No amount of health care will prevent everyone’s ultimate death.

* Comprehensive, unlimited, non-subsidized private-market coverage isn’t possible for people with severely impaired health.

* The private health market can provide limited non-subsidized health insurance, such as protection from accidents, to even health-impaired individuals.

* Individuals who can afford to do so and who take good care of themselves should be able to “buy up” to better coverage. People have the option of buying up for everything else in life.

Discussion of these principles is lacking from most of the current health insurance debate. If society can intuitively understand how similar principles apply to health insurance, then they should be able understand the principles in the health insurance context. We need to initiate the debate.

Three Secrets You Want to Know to Playing Your Best Tennis

There are so many millions of people who play tennis everyday throughout the world. They play in public parks, country clubs, private tennis courts, and anything in between (i.e. against a wall).

Tennis is the only sport I know that begins with love and is a sport for a lifetime. I remember the great Sugar Daddy, Bobby Riggs who said in one of the many tennis clinics he taught, “Tennis is a sport you can play from cradle to grave.”

Why do people enjoy playing tennis? I believe because it is a classy sport. It is a sport you can play for as short as 15 minutes or as long as 3 to 5 hours, it all depends on your ability level and motivation. I was fortunate enough to play professional tennis. I had a world ranking in the ATP Tennis of 662 in singles and 457 in doubles. I had wins over players ranked in the top 200 in singles in the world and wins over players ranked in the top 50 in doubles.

In this article, I want to provide you with the three secrets to playing your best tennis. Of course, there are more than three secrets, but three secrets are enough to help you play better tennis.

First, there is no substitute to practicing, practicing, over and over and over again. Practice is the seed to building confidence! When you practice in addition to playing tennis regularly, you will feel a sense of accomplishment, you will feel that you deserve to win because you know for yourself that you have been investing the time to improve your game. So, practice at least two to three hours extra a week if you are a club tennis player. If you are a tournament player you should practice at least 15 hours a week, which equals to 3 hours a day, 5 days a week.

Second, visualize yourself hitting the tennis ball perfectly. Imagine yourself in the movie screen of your mind that you are moving side to side, running forward and backward, hitting each ball with perfect accuracy. Visualize your forehand, backhand ground strokes being hit over the net with 3 to 5 feet clearance. Picture every stroke you hit with perfect ball control. See each of your tennis strokes, feel each tennis stroke, and experience each and every tennis stroke you hit with confidence. Repeat this each time pre and post tennis game or match. By so doing, you are training your mind or inner game to manifest itself in your outer game or physical game. Tennis is a mind game once you have learned all the fundamentals of each tennis stroke.

Third, watch the big players or your favorite tennis pro on the ATP or WTA tour and copy their tennis style. Watch YouTube videos or television footage of your favorite player over and over again until you emulate their movements. One of my favorite players to watch when I was playing competitive tennis was John McEnroe (USA) and Guillermo Villas (Argentina). I use to watch McEnroe’s serve and literally copy his serve. I would watch Villas hit his ground strokes over and over again until I was mirroring Guillermo Villas’ ground strokes. It was usually after watching these great tennis players in a grand slam event that I mirrored their mannerisms. After all, why event the wheel, copy what the big players do and do the same. Of course, as your tennis game matures, you will begin to develop your own style and preferences.

So, there you are the three secrets you want to know to playing your best tennis! Remember, there is no substitute to practicing each and every day in addition to playing regularly tennis matches. Begin today if you do not already do so; visualize yourself hitting the tennis ball with each and every tennis stroke perfectly until you are confident in your mind and in your heart that you can execute it with ease and confidence. And, finally, do not reinvent the wheel, copy the tennis players on the tennis tour by watching their tennis until can have someone say on the side line, “Hey, that person hits the ball like Roger Federer or Maria Sharapova.” Before you know it you will be winning your club tournaments and filling your shelves with trophies.

What did they think of your energy level, passion and commitment to what you’re saying? What do they think could be improved? Also practice in front of a mirror. This will help you rehearse standing up and let you see what needs to be improved. The more you rehearse standing up, and the more familiar you become with your material, the more comfortable you will be with your public speaking. This one tip alone can dramatically improve public speaking skills.

3. Put your presentation as a “rough draft” on a micro cassette recorder. Listen to it while you’re driving to and from work, picking up the kids, or driving to the supermarket. I know what you’re thinking. “I don’t like how I sound on audio tape!” I realize that. But this is a convenient way to learn material quickly. Again, where do you sound convincing, passionate and interesting? What parts of your presentation do you think need work? The audio cassette recorder is another objective ally.

4. Visualize yourself giving a successful presentation. Better yet, if you know the exact room you’ll be speaking in visualize that, too. Olympic athletes practice visualization. It works for public speaking as well.

5. To improve public speaking skills, do a dress rehearsal. Before I conduct a keynote speech or breakout session at a conference, I take a peek inside the ballroom. If the room’s empty, I’ll go in and practice a “dry run.” This is something I try to do in full dress rehearsal mode. If the ballroom is being used until morning, I go in that morning.

Rehearsing in the exact room will give you an edge in feeling more comfortable and conquering fear of public speaking. This is one of the biggest public speaking secrets of many great public speakers. Guaranteed! If you’re usually presenting in small meetings or groups these same techniques work.

6. Are your notes and visual aids large enough print that you can see them standing up? Your notes should be “fast food for the eyes.” Never read your presentation. You’re the presenter. Your audience expects you to be the expert. You want that connection with them. A secret to effective public speaking is bonding with your audience through stories, activities, appropriate humor and understanding their needs ahead of time.

7. Stand up when speaking in public. Even if only three people attend, it sends a signal that says, “You’re so important that I’m going to stand even for the three of you.” According to a University of Minnesota study, when you stand in presenting your ideas, you are more believable, credible and persuasive.

8. Stand “center stage” when presenting your most important point. It grabs the audience’s attention. The rest of the time you can move around as long as you aren’t nervously pacing like a lion. Again, that’s where the videotaping helps!

Visual Aids and Public Speaking: Less is More

9. With public speaking and visual aids, less is more. Don’t use more than three or four colors per slide. Otherwise, people start focusing more on color and less on content. Use graphs for sales figures or sets of numbers showing a trend over a period of time. Graphs are pictures that increase retention and comprehension. No more than two or three lines on a graph. Use pie charts for market share, budgets, expenses analysis, income sources and the like.

10. Avoid slides with yellow, pink or orange print. They don’t show up well. Dark blue, black or any other dark color is better. White is okay with a darker background. Red stands for negatives like “danger,” or “warning.” Only use red to indicate problems, your competition, stopping or something similar.

11. Public speaking and eye contact: Approximately three to five seconds of eye contact per person with a small or medium sized group. Many public speakers make the mistake of using their visual aids AS their presentation. A key point in persuading your audience is establishing connection and credibility.

12. What if someone in your audience doesn’t like what you have to say because they don’t like the product or service you’re selling? Or what if you fear public speaking because you often have to deliver bad news? Know and research your audience ahead of time. What will be their biggest objections? THINK AHEAD when planning your presentation how you’re going to handle those issues.

13. Never lose emotional control. Often, these difficult people are trying to rattle your cage. They want control. And they want it in front of others. What if they continuously discount what you’re saying? Tactfully respond to them at first. After a while, say something like, “You bring up a good point, and yet, due to time constraints see me at the break and we’ll discuss that privately.” Your audience will be looking to see how you handle the situation.

Blogging for Candidates 101: Nuts and Bolts

A “blog” is simply an internet (web) log. Blogs are created for personal or professional use. They may promote a product or service, or merely serve as a personal online journal. There are currently just over four million blogs today, with a new blog born every seven seconds.

The problem of cocooning candidates

Today, we work and live in an era of heightened cynicism and secrecy. Isn’t it much harder than it used to be to call into a company and attempt to speak with a candidate by telephone? Central voice mail systems have grown more sophisticated and guarded. And even when recruiters are able to finally speak with an actual live person, it’s often a reluctant administrative assistant or receptionist. Finally, if you are fortunate enough to be transferred to your candidate, more often than not you are greeted with that person’s individual voice mail recording. It has become de facto practice by many professionals today to simply leave their “do not disturb” function turned on for most of the work day. Later, they will screen and return external calls at their discretion.

To address these changes, in recent years legitimate e-mail and web site marketing was considered a non-threatening (and somewhat successful) way to reach these “cocooning candidates.” These are in fact still viable tools, but there costly learning curves associated with them.

Some recruiters have gone back to launching traditional direct mail campaigns. However, this can be an expensive and time consuming proposition to undertake with regularity. Success may be mixed until you find just the right combination of style and timing.

Moreover, most outreach marketing attempts essentially are competing for a candidate’s attention today. Consider this:

• Only 29% of eligible job seekers visit the “Big Three” (Monster.com, Hotjobs.com, CareerBuilder.com)

• The average consumer is exposed to about 3,000 marketing messages per day.)

• 85% of people conducting research are using the internet.)

What can blogging do for you?

Recruiters (or researchers) who seek candidates for open positions, or to profile candidate requirements for the purpose of building a network pipeline, there are two ways we can use blogs:

(1) The first way is to start your own blog and attract top talent to your blog site. This is not an overnight marketing cure-all, and it will require some patience. But cultivating a readership of professionals in your niche field is a highly effective means of reaching candidates who would have never otherwise learned about your career openings. The key to making this work is to learn to attract a specialized readership to your blog, just like a beacon in the night. You don’t need millions of readers…Just hundreds of the right readers.

(2) The second approach is to search other existing blogs and develop contacts and relationships. This is a perfectly legitimate means of networking, as any internet page is essentially public domain information. Based on posting activity and interactions you initiate, you can easily develop in-roads and find more candidates in less time.

Let’s talk about both approaches.

You want to start your own blog. Now what?

How much time do I need to spend on my blog? What do I write about?

By their very nature of being near real-time, dynamic logs about life, blogs are frequently published mediums. I publish my blog at least three times per week (sometimes more often if I have more to say).

If you are going to take the time to create an effective blog, and develop a regular reading audience, I suggest blogging at least twice every week.

How do I decide what to blog about?

Who is your audience? What message are you trying to convey? Are you recruiting for new business clients? Looking for joint venture relationships? In need of candidates who are in short supply in your market place? Are you selling products or services, or promoting your company brand?

The first step you should take is to decide on what your intended focus will be. If you write about what’s wrong with our political system one week, then the following week write about outsourcing IT services to India, then in week three prattle on about how beautiful your home town is in the fall, your readership will not feel a sense of community or predictability about you. This completely defeats the purpose of building trust and credibility! You would be better off making cold calls if you continue on this path.

One of my blogs (The Hiring Insider, http://www.hiringinsider.com) shares tips and tools to help line managers, corporate recruiters, human resource professionals, etc. learn to make better hires faster (or so I hope). Week in and week out, I try to maintain a specific, consistent thread of helpful information that builds upon previous week’s blog entries. However, a new reader should be able to jump in without any trouble at any time, so remember this as you plan your writing agenda. My other blog (http://jobsblog.blogspot.com) provides tips to job seekers in the life sciences field who are confused about who to trust in the pharmaceutical job market, and what practices to engage in and avoid in order to land the best possible job for their skill set.

As you craft your blog entries, constantly think about who, what, why, where and how. Be sure to ask yourself where you are headed over time with your blog topics. Try to stay theme-related.

How do I choose a service to blog on?

There are many services available. For beginners, I recommend using blogger.com (Pyra labs). Google acquired blogger.com in February of 2004 ([http://weblog.siliconvalley.com/column/dangillmor/archives/000802.shtml]) and is really enhancing its features. It’s also FREE. Yep, you can register for an account in less than ten minutes and be up and blogging. Other great tools include Typepad (http://www.typepad.com/) and Movable Type (http://www.movabletype.org/).

Again, the most important ingredients are publishing good content, and establishing and maintaining a base audience.

However, you aren’t done yet. Now that you’ve come this far, you need to promote your blog and get yourself out of the starting gate.

Begin by submitting your blog to the blog search engines. There are several hundred of these, and they are growing by the day. It is somewhat arbitrary which ones you choose to register with. However, at a minimum, I recommend that you register your site with at least the following:

What if I don’t want my own blog?

If you decide instead that you want to search other blogs to source candidates, you can do any or all of these three steps:

First, search for blogs based on what skill sets you are seeking (Oracle blogs, copywriting blogs, programmer blogs). Secondly, read the blog entries and see who is posting comments. Third, most bloggers have “blog-rolls” on either the left or ride side of the page. These cite other blogs that they like, and are usually related to the subject matter on their own blog. Resourceful recruiters will take advantage of these links and research them as well. You will also find blogs they may like or recommend which are unrelated.
